Any clinical note template (but not clinical tool template) added to a patient intake form will give you the option of appending a consent checkbox or signature box where patients can sign using their mouse or touch device. This allows you to easily record a patient's signature or electronic consent to treatment and to terms and conditions.
To add a consent form to your patient intake form:

Create your consent form as a clinical note template. You do not need to add a section for the patient to sign or agree (Halaxy will do this automatically when you include it in your patient intake form).

When creating or editing your intake form, click Add Another Template and choose your consent form. You will be able to choose any clinical template you have created. Only clinical note templates (not tool templates) will give you the option to add a signature or consent checkbox.

In the Consent Type field, select whether you wish to include a Signature box where patients can sign using a mouse or touch device, a Checkbox that patients can tick to record consent, or None. Tick the Mandatory checkbox to make providing a signature or ticking the consent checkbox mandatory.

Click Save.
RESULT: Your consent form will now be included as part of your patient intake form. If you selected to include a checkbox, there will be a checkbox labelled "I accept the terms listed above" that patients can tick. If you selected to include a signature, there will be a field for patients to sign, as well as a place for patients to type their name.
If you made the consent type mandatory, patients will not be able to submit the intake form without signing or ticking the checkbox.


You can view a patient's completed intake form to review their acceptance to consent forms and terms and conditions. A timestamp will show exactly when the patient signed or ticked the checkbox. For signatures, the signature will also be included on the clinical note that is created in the patient's record.
