Data sets go hand in hand with clinical questions. A data set is the set of possible responses to a single choice or multiple choice question - for example, if you create a question relating to frequency, you can create a data set with responses such as "Never", "Sometimes" and "Often" to use as possible answers.
You can reuse data sets for multiple questions, saving you time. For example, a data set with the possible responses being 1 to 10 could be used on any question where you want a score between 1 and 10.
Clinical tool templates and customised patient profiles allow you to add your own questions to capture data, in addition to the questions already in Halaxy's database.
To create a data set:

Click Settings > Clinical, then click the Data Sets tab.

Click Add Data Set in the top-right of the page.

The New Data Set page appears. Give the data set a name and description.

In the Answer Options section, click Add Value to add possible answers.

New fields will appear allowing you to add the Display Value/Answer Title and a score. In the Display Value field, add the wording of the answer (e.g., if the question is about frequency, you would use values such as "Never", "Sometimes" or "Often).

(Optional) Add a score. When creating a clinical questionnaire, the scores of various questions can be tallied to provide an assessment. If the question does not require a score, skip this step.

Repeat Steps 4-6 for each possible response you want to add to the data set.

Click Save.
RESULT: The data set is now ready for use and can be assigned to any single choice or multiple choice questions.
To assign a data set to a question, create any single choice or multiple choice question, then select the data set you would like to use as the possible responses. You can then add the question to clinical tool templates or customised patient profiles.
The Clinical Questions report allows you to track patients and their answers to clinical questions, including the date they answered them.
To run a clinical questions report:

Click Reports > Patient.

Click New Report at the top-right of the page.

In the Report Type field, select Clinical Reports.

In the Template Question field, type part of the question you are looking for and select it from the drop-down menu. If applicable, a new field will appear where you can optionally filter by the answer based on the answer data set.

(Optional) Add any other filters as desired.

Click Run.
RESULT: A report will be generated that shows patients who have answered the question, what their answer was and when they answered it.
