Guide to patient social history

Patient social history provides background information on a patient that can be helpful for providing appropriate treatment. In Halaxy, the Social History form has its own tab in the patient’s profile and can be fully customised.

Overview

With the patient social form, you can record background information including:

  • Active problem list

  • Past medical history

  • Medications

  • Allergies

  • Family history

  • Social background

  • Economic status

  • Demographic information

To record information, you can manually fill in the form yourself, or you can include it in patient intake forms for your patients to complete.

Having its own tab in the patient’s profile allows the Social History page to always be accessible for quick reference when treating your patients.

Enable the Social History form

The Social History page is disabled by default. You must enable this feature before you can start using it.

To enable the Social History form, follow the steps below

  1. Click Settings > General.

  2. Under the Patient Portal section, next to Social history form, click the Icon-Edit.svg pencil icon. (Note: Make sure you are clicking in the Patient Portal section – not the Clinical Settings section.)

  3. In the pop-up, select Enable.

  4. Click Save.

  5. The Social History tab now appears in each patient's profile and can be accessed from there.

Edit the Social History form template

To help you get started, Halaxy has populated the default social history form template with a number of sections and questions, which you can add to, edit, or remove to suit your needs.

To customise your Social History form, follow the steps below.

  1. Click Settings > General.

  2. Under the Clinical Settings section, next to Social history form, click the Icon-Edit.svg pencil icon. (Note: Make sure you are clicking in the Clinical Settings section – not the Patient Portal section.)

  3. In the template editor, customise the form as needed.

  4. Click Save.

Tip

The Social History form template works the same as creating a clinical tool. See our support article on clinical tools to learn more about customising this form.

Note

Adding questions to your Social History form also adds the questions for any patients who have previously completed this form. If you delete questions from the form, the questions remain on the Social History tab for patients who have already answered them.

Record answers in the Social History form

There are two ways to record information for your patient's social history.

  • Automatically via patient intake form

    1. Click Settings > General.

    2. Under the section Clinical Settings, next to Patient intake form, click the Icon-Edit.svg pencil icon. (Note: Make sure you are clicking in the Clinical Settings section - not the Patient Portal section.)

    3. In the list, select the patient intake form to open it.

    4. In the Intake Template section, under Sections, tick the checkbox Include Social History information.

    5. Click Save.

    When your patient completes the intake form, the Social History tab in their profile is automatically populated with their answers.

  • Manually via patient profile

    1. Open the patient profile, then click the Social History tab.

    2. On the top right, click Edit.

    3. Complete the form as needed.

    4. Click Save.

    The social history form answers are saved. Click the Social History tab again to refresh the page and view the completed form.
