Who can do this?
All users with Clinical Settings permission
Bring the forms your practice already uses into Halaxy, from intake questionnaires to outcome measures and assessment checklists. Upload a PDF of the form and Halaxy AI drafts the sections, questions, answer types and answer options for you, so there's no need to retype it question by question.
Halaxy AI also reuses the data sets your practice already has, so your answer scales stay tidy. You review the draft, make any changes and save it. Nothing is created until you save.
Creating a clinical tool using AI is free and doesn't use Halaxy credits.
To learn how Halaxy AI handles your form, see FAQ: AI in Halaxy.
Halaxy AI takes care of the setup, so all you need to do is review the draft and save it.
-
No retyping: Halaxy AI reads your form and drafts the sections, questions, answer types and answer options for you.
-
Your data sets, reused automatically: when your form repeats an answer scale, such as None, Mild, Moderate, Severe, Halaxy AI matches it with your practice's existing data sets instead of creating duplicates. Each data set you add is reused by later uploads, so your library builds up as you go.
-
Quick to generate: Halaxy AI usually takes 40 seconds or more to read a form, much quicker than rebuilding a long form question by question.
-
The forms you already use: upload a PDF of up to 5 MB, such as an intake questionnaire, outcome measure or assessment checklist. Word documents and image files aren't supported.
-
One form, one clinical tool: each PDF becomes its own clinical tool, so upload your forms one at a time.
-
Built for clinical tools: Halaxy AI creates clinical tools, such as forms and questionnaires. For a text-based template, create a clinical note template instead.
With just a few clicks, turn a PDF of your form into a draft clinical tool.
-
Click Settings > Clinical.
-
On the top right, click Create using AI.
-
The template builder opens with the Create with AI pop-up. Click Choose PDF and select your form. You can also drag and drop the PDF into the pop-up.
-
Click Generate template.
Halaxy AI reads your form and drafts your clinical tool for you. When the draft is ready, it opens automatically in the template builder, with a Drafted with AI banner that shows the name of your PDF. Keep the page open while Halaxy AI works.
Note
If Halaxy AI can't find any questions in your file, for example because it isn't a form, the pop-up lets you know. Choose a different file and try again.
You stay in control of what's saved. Review the draft against your original form, make any changes, then save it. A Drafted with AI banner at the top of the builder reminds you to check every field, question and answer type.
-
On the Info & Metadata tab, check the template details. The template name comes from your PDF file name, so update it if needed.
-
Click Next: Sections & Questions and check each section and question against your form:
-
Answer types: check each question's answer type and answer options.
-
Data sets: each question Halaxy AI drafted shows a New question tag. Click a question to see its answer type and data set. A data set Halaxy AI suggested, such as "DASS 21 options", is created when you save. To review a question before saving, click the
pencil icon on its tag.
-
Final touches: add or change any question to suit your practice.
-
-
Click Next: Scoring. If your form is scored, set up scoring.
-
Click Save template.
Your clinical tool, its questions and any new data sets save together in one step. The clinical tool is Active and appears in your template list in Settings > Clinical, ready to use in clinical notes. From then on, it works just like a clinical tool you build yourself, so you can edit it whenever your form changes.
Important
Nothing is added to your template list until you save. If you leave the page before you save, the draft is discarded.
Tip
Upload a blank copy of your form. Halaxy AI only needs the questions and answer options, so there's no need to include any patient details.
