When a patient submits a completed intake form, you will receive a notification in Halaxy and the patient's profile will be automatically updated with the data from the form.
You can view a patient's completed intake form in two ways:
From the patient's profile
From the patient's clinical notes record
By having this information readily available, you can stay knowledgeable on the patient you are treating.
You also have the option to print the completed intake form in its entirety.
If a patient needs to complete another intake form, any existing information in the patient's profile will be prepopulated in the new form, which the patient can then overwrite and update.
Go to the patient's profile, then the General tab.
Under the Contact Details section, click the Send intake form icon.
The patient intake form pop-up appears. Scroll down to the bottom and click the plus icon to expand the History section.
