Getting patient consent is a key part of providing care. With Halaxy, you can create digital consent forms that make this process quick and easy. Whether it's for treatment, telehealth, or agreement to terms, you can customise forms to suit your practice and have patients complete them online, even on their mobile device.
In Halaxy, consent forms are sent via the intake forms feature - whether as part of your practice's intake form or as a standalone form. Consent can be captured by requiring the patient to tick a checkbox or adding their signature.
When the patient submits the form, a timestamp shows exactly when the patient signed or ticked the checkbox. The completed form is also saved in their clinical notes.

Step 2: Add your consent form template to a patient intake form
Your consent form can be part of your intake form or it can be a standalone form.

Click Settings > General.

Under the section Template Settings, next to Patient intake form, click the
pencil icon. (Note: Make sure you are clicking in the Template Settings section - not the Patient Access section.)

If you are adding the consent form to an existing intake form, click the intake form in the list. If you want to send the consent form by itself, click Add Form in the top right.

Under the Manage Templates section, click Add Template and select the note template you created as your consent form.

For Consent Type, select how you want to capture your patient's consent - checkbox or signature.

(Optional) Next to your selected consent type, tick the Mandatory checkbox to make consent required. If this checkbox is ticked, patients cannot submit the form without providing consent.

(Optional) If you are sending a standalone consent form, untick and remove all the other sections in the intake form.

Click Save.
Your consent form is ready for sending. For standalone consent forms, you can send them the same way you send intake forms, which can also be automated.
If you selected the checkbox consent type, the form automatically includes a checkbox labelled "I accept the terms listed above".
If you selected the signature consent type, the form includes an area for patients to sign using their mouse or touch device. There is also a textbox for patients to enter their name.
