How can I record a patient's consent/signature for treatment or terms and conditions?


The easiest way to do this is to create a consent form as a clinical note template, then add that template as part of your patient intake form and adding a consent type. You can choose to include a consent checkbox or signature field that patients can sign with a mouse or touch device. You can also choose to make signing or ticking the checkbox mandatory for submitting the intake form.

Another way of doing this is to create a template question that can be re-used on any clinical template that you create, including patient intake forms, clinical tools or the Social History form, all of which will form part of your patient's electronic health record.
